Balanced Salt Solution Assisted Intraocular Lens Implantation in Phacoemulsification Surgery: Intraocular Pressure and Endothelial Study

INTRODUCTION: Purpose: To evaluate the effect of balanced salt solution (BSS) assisted IOL implantation on postoperative IOP and endothelial cells and to compare BSS assisted intraocular lens (IOL) implantation with using ophthalmic viscosurgical devices during IOL implantation.

METHODS: Methods: 52 eyes of 40 patients (25 female, 15 male) with cataract and undergoing phacoemulsification surgery with BSS assisted (26 eyes) or viscoelastic assisted IOL (26 eyes) implantation were evaluated. BSS assisted IOL implantation was performed with the aid of irrigation cannula and BSS without using viscoelastic. Viscoelastic assisted IOL implantation was performed standardly that IOL was implanted with the aid of cohesive viscoelastic. IOP and endothelial changes after phacoemulsification surgeries and the surgical time were noted and compared. And also, the eyes were evaluated in terms of cataract density preoperatively and phaco parameters peroperatively.
RESULTS: Results: The mean age was 67.5±7.8 years in BSS assisted group and 67.8±9.4 in viscoelastic assisted group. The mean preoperative IOP, postoperative IOP at 1. day, 1. week and 1. month was 14.2±2.3, 14.7±3.1, 13.2±1.5 and 13.8±2.7 respectively for BSS assisted and 14.1±2.9, 19.1±3.4, 13.8±3.1 and 13.2±2.9 respectively for viscoelastic assisted IOL implantation. IOP increase was significantly higher at first day in viscoelastic assisted IOL implantation group (p=0.007). The surgical time was 12.3±2.1 minute in BSS assisted group and 14.6±3.1 in viscoelastic assisted group (p=0.035). The endothelial changes were not statistically significant between both group (p=0.88).

DISCUSSION AND CONCLUSION: Conclusions: IOL implantation using BSS in phacoemulsification surgery is reliable technique, and this method provides significant shorter surgery times and much lesser postoperative IOP increase. The lesser increase of IOP after cataract surgery might be useful for glaucoma patients.
